A complete medical history of your camper is the best assurance that we can offer the proper care for your camper during the summer.
Please complete all medical forms in your CampInTouch Account. These are:
1) Camper Health History Form
2) Doctor-Completed Medical Forms (Includes physical, immunization history, medication list, and physician orders)
Note: If your camper is not immunized, we require a physician-signed ‘Immunization Exemption Form.’ The forms listed above are available online in your camper’s account, which you access via the “Parent Login” link at the top of our web page, www.edenvillagewest.org. Call us with any questions.
